SUMMARY One of a pair of monozygotic twins presented with a torsion of the testis. The anatomical abnormality, a high investment of the tunica vaginalis with inversion of the testes, was bilateral. The testes of the other twin were explored and found to have the same predisposing abnormality. Prophylactic orchidopexy was performed. Further studies of similar cases are required to establish the degree of concordance of the predisposing anatomical abnormality in monozygotic twins and hence a rational policy for management of the monozygotic twin of an established case. On the basis of other congenital abnormalities the concordance rate will probably be between 25 and 50 per cent. Meanwhile it would seem wise to warn the parents that the other twin should seek medical advice at the slightest suggestion of symptoms of torsion.
