SUMMARY The effect on the radioisotope renogram of a mannitol or frusemide diuresis has been studied in 57 patients with ureteric obstruction. The characteristic gross delay in the excretory phase either remained unchanged or became considerably less marked. In cases where improvement did not occur the clinical and radiological findings were consistent with a severe degree of ureteric obstruction whereas in those showing improvement the findings suggested a milder degree of ureteric obstruction or a residual dilatation of the upper out‐flow tract. Included in this study were 30 patients with acute renal colic due to ureteric calculi and in 27 of these this method correctly predicted whether or not the calculus was still present in the ureter.
