SUMMARY Hypertrophy of a renal papilla is a rare abnormality with no distinctive clinical features. It can be demonstrated by contrast radiography but only nephrectomy or partial nephrectomy will give the final diagnosis and distinguish it from a neoplasm in the renal pelvis. A case is described and the literature is reviewed.
