SUMMARY Three cases of cystic disease of renal pyramids are presented. The biochemical features of these cases were studied and their significance is discussed. One of the cases required a nephrectomy and the histology of the kidney showed the classical features. The different clinical presentations of cystic disease of renal medulla are discussed and it is suggested that the term medullary sponge kidney be replaced by cystic disease of the renal pyramids.
