SUMMARY Intraperitoneal rupture of hydronephrosis is an uncommon condition. Pre‐operative diagnosis is difficult, but there is usually a clear indication for emergency laparotomy. Early drainage is essential for survival, and is preferable to emergency nephrectomy, allowing subsequent full investigation of renal function with the possibility of elective conservative surgery in some cases.
