SUMMARY 1. Large amounts of residual urine in the bladder can be eliminated or considerably reduced by invaginating the apex and dome of the bladder, provided the cause of the condition has been removed. The muscles of the invaginated portion must be firmly sutured together. In order to carry out this operation the peritoneal cavity must be opened. 2. It is believed that the muscles of the invaginated bladder wall have a detrusor action, which is in part responsible for the good results obtained. 3. The operation has a use in chronic retention of a neurological origin, provided the cause is removed, e.g. , excision of a tumour of the cauda equina. The operation corrects the result of chronic over–stretching of the muscle fibres, mucous membrane, etc., which has resulted. It should have an increasing use in this type of case.
