SUMMARY A case of metastatic adenocarcinoma of the colon masquerading as bilateral intraluminal ureteral papillomas followed by bilateral hydronephrosis and urasmia is reported. Tumour involved all layers of the ureteral wall, including the mucosa. Antegrade pyelography is again emphasised as a tool in diagnosing the site and cause of ureteral obstruction in cases where retrograde studies are inadequate. Intrarenal pressure studies obtained during antegrade pyelography may predict the relative function of the kidneys. Cutaneous ureterostomy in situ may be a life–saving operation in a severely ill patient with ureteral obstruction. A definitive form of urinary diversion later is almost always necessary.
