SUMMARY The results of the Boari flap operation for replacing the lower ureter following injury are described in twelve patients. Reflux was present in nine patients, but, in spite of the presence of a urinary tract infection in four, no deterioration in renal function has occurred over a two–to–twelve–year follow–up. Two patients have no ureteric reflux, one of whom had a submucosal reimplantation of the ureter into the flap; the other has a partial stenosis at the anastomosis. The remaining patient has pyelonephritis on the operated side and hypertension, but it is not known whether she has ureteric reflux.
