SUMMARY First, the concept of giant hydronephrosis is established; this is when there is not a single pyelectasis, but transformation of the kidney into sacs with thin walls. Three different operations are next examined: renal folding, following a personal technique, which gives good results when there is precise indication; total substitution of the ureter with a vesical tubular segment, which has given bad results; and total substitution of the ureter with an ileal loop, which is the method lending itself to most extensive application.
