SUMMARY Seromucoid estimations have been carried out on ninety patients suffering from prostatic obstruction. Benign prostatic hypertrophy can be distinguished from carcinoma of the prostate in the great majority of patients by this method. The results of seromucoid estimation in carcinoma of the prostate are compared with those of serum acid phosphatase and seem to provide a more accurate indication of diagnosis and a more useful biochemical index of hormonal control. It is suggested that the divergence between seromucoid and serum acid phosphatase levels might provide a useful indication of the degree of cellular differentiation, invasiveness, and hormone sensitivity in prostatic carcinoma. We wish to thank Dr P. D. Stewart, Dunbartonshire Area Pathologist, for classifying our biopsy material and Mr A. Crooks of the Vale of Leven Hospital, and Mr W. Plenderleith of the Royal Alexandra Infirmary for carrying out the biochemical estimations.
