SUMMARY A case of hemipyonephrosis in a duplicated kidney, simulating an extrapelvic mass in a normally functioning kidney, is described. Difficulties in diagnosing the underlying pathology pre‐operatively are discussed, and reference is made to similar recorded cases. I wish to thank Mr Pelham Warren for permission to publish this case and for advice in its preparation; and the Photographic Department, St Margaret's Hospital, for the illustrations.
