SUMMARY Analysis of lower urinary tract dysfunction due to neurological disease by means of combined simultaneous cinefluorographic cysto‐urethrography and direct cystometry indicates that patients with reflex neurogenic bladders have high resting intravesical pressures with further increase at the time of reflex contraction and that this contraction is not sustained. The high pressures result from hypertonicity of the detrusor and lack of co‐ordination of the outlet mechanism. Abdominal straining and suprapubic pressure produce high intravesical pressures and may lead to the sequela? of bacteremia and ascending urinary tract infection when used to effect emptying of autonomous‐type neurogenic bladders. As a result of these findings, methods of management which remove the danger of high intravesical pressures are recommended.
