SUMMARY An operation is described for the cure of severe strictures which extend posteriorly to the region of the sphincter. The operation is in two stages. Except in complicated cases, diversion of the urine is unnecessary. The length and original situation of the urethra are maintained. No tissue is excised. A suitable calibre of the reconstructed urethra is arranged and the risk of a subsequent stricture reduced to a minimum. The blood supply of all the tissues concerned is interfered with as little as possible. The complications which may occur are described, and their prevention and correction discussed.
