SUMMARY 1. The movement patterns of the normal pelvi‐ureteric segment as seen on cinepyelography, at operation or during perfusion, suggest that co‐ordinated contractions of the renal pelvis and upper ureter are associated with a uniform distribution of mixed muscle spirals at the pelviureteric junction. 2. In congenital hydronephrosis pelvi‐ureteric inco‐ordination can be demonstrated at operation or during perfusion by raising the intrapelvic pressure, and may be associated with a predominance of longitudinal muscle fibres at the pelvi‐ureteric junction in place of the normal spirals. 3. The part played by this defect in the genesis of the various clinical types of hydronephrosis is outlined and a possible mechanism for the production of the pelvic dilatation is suggested.
