SUMMARY Certain patients with male hypogonadism characterised by hyalinisation and fibrosis of the seminiferous tubules have female nuclei. They appear to be instances of an almost complete sex reversal that dates from an early stage of embryonal development. The indifferent gonads apparently develop as imperfect testes, rather than ovaries, and the androgenic activity of the embryonal testis‐like gonads directs sexual development in the male direction. It is important to recognise these patients in order to establish the cause of sterility when this problem is under investigation, and to take advantage of the benefits that accrue from androgen substitution therapy.
