SUMMARY 1. Sixty‐three tumours were treated by perurethral fulguration without previous biopsy more than five years previously. Of the thirty‐two patients with sessile turnours fifteen died rapidly from bladder disease. To help to distinguish those sessile tumours unsuitable for diathermy, perurethral biopsy is advocated. Tumours not responding rapidly to fulguration should have more radical treatment at an early stage. 2. Sixty‐seven tumours were treated by suprapubic fulguration without previous biopsy more than two years previously. All tumours were considered non‐infiltrating or just beginning to invade the bladder wall. The immediate results on the histologically simple group have been satisfactory. Fourteen of twenty‐six patients with malignant turnours who left hospital died rapidly of bladder tumour. The histological reports indicate that both cystoscopy and direct palpation of the tumour are unreliable methods of detecting early infiltration. Adequate perurethral biopsy is advocated before the bladder is opened.
