Answer: There is no hard-and-fast ceiling for any individual patient but, as we saw in the 4-T study, people with A1C levels greater than 8.5% on dual oral therapy were less likely to achieve A1C levels below 7%. A1C levels above 8.5% suggest that a more complex insulin regimen is needed, containing a component of prandial coverage. The most important lesson is that treatment strategies should be re-evaluated before A1C levels become that high.
