Posterior colpotomy for transvaginal specimen retrieval after laparoscopic colectomy. Video demonstration of technique and anatomical landmarks in Pelvis

Advances in laparoscopic colorectal resection have already demonstrated benefits in surgical recovery. The improvement in recovery is in part related to the reduced trauma to the parietes of the abdominal wall. Retrieval of the resected specimen has required a short laparotomy. Posterior colpotomy has been used in gynaecological surgery to provide access to the abdomen for specimen retrieval after subtotal hysterectomy and oophorectomy, removing the requirement for laparotomy completely. This technique represents progression encompassing many of the benefits in Natural Orifice Transluminal Endoscopic Surgery (N.O.T.E.S). We have reported the use of a posterior colpotomy in the retrieval of a colonic specimen after laparoscopic right hemicolectomy and are currently undertaking a prospective randomised controlled trial of specimen retrieval via mini‐laparotomy or posterior colpotomy. This presentation aims to demonstrate the surgical anatomy encountered where the technique is used by means of video footage and viva voci. We discuss potential pitfalls for a colorectal surgical audience new to a gynaecological technique which has been in practice since 1951. A short video demonstrates the technique and scope of its approach.
