Summary A 13‐week‐old baby weighing 5.7 kg donated bone marrow for transplantation to his 4‐year‐old brother with metachromatic leucodystrophy. This required aspiration of 260 ml of marrow, 60% of estimated blood volume. The procedure was successfully managed with non‐invasive blood pressure monitoring and continuous replacement of blood against measured losses, combined with temperature and arterial blood gas measurement per‐operatively. No complications occurred with the donor and the recipient was successfully engrafted.
