We present a rare case of a 61‐year‐old Japanese woman with an ovarian leiomyoma. That was detected during a mass screening. To our knowledge, this is the first report of an ovarian leiomyoma found during a mass screening. We performed a total abdominal hysterectomy and a bilateral salpingo‐oophorectomy. The tumor originated in the right ovary, and it did not connect with the uterus. The smooth muscle origin of the tumor was detected by immunohistochemical staining with antibodies to vimentin, desmin, and smooth‐muscle actin (SMA).
