Efficacy of γ‐Globulin Infusion Followed by Platelet Transfusion in Patient with Aplastic Anemia during Pregnancy: A Case

Abstract A case of aplastic anemia with positive antiplatelet antibody during pregnancy is summarized. A single 400 mg/kg dose of γ‐globulin was given intravenously, on August 9, at 35 weeks' gestation, soon after spontaneous rupture of the membranes. During injection of γ‐globulin, labor began spontaneously and a viable 1,980 g infant was delivered by spontaneous vaginal delivery. After parturition, vaginal hematoma developed. Following the γ‐globulin injection, a 13‐unit platelet concentrate, 250 m l of platelet concentrate produced at the Shuto Hospital and 1,000 m l of fresh blood were injected. After these blood transfusions, bleeding from vagina subsided. Twelve hours later the platelet count was 33,000/mm 3 . It is suggested that γ‐globulin infusion followed by platelet transfusions may be a practical way to control hemorrhage during parturition in women with positive antiplatelet antibodies and aplastic anemia.
