Mesenteric cysts are encountered in all age groups. These cysts present with bizarre clinical findings if large enough to be symptomatic, such as pain, a mass and acute or chronic intestinal obstruction. We experienced a rare case of a large chylous cyst of the mesentery. She presented with a huge abdominal swelling equivalent to a 36 weeks gravid uterus simulating an ovarian cyst. The cyst was successfully shelled out of the mesentery and excised. She made an uneventful recovery. Because of the infrequent pre‐operative diagnosis, a report of the case is presented.
