The sinusoidal fetal heart rate pattern (SHR) was studied by performing the simultaneous non‐stress test (NST) in twin pregnancies. The NST led to a diagnosis of SHR according to Maeda's tentative criteria in 3 sets of twins. SHR was recorded in 3.2% of our twin pregnancies. All cases were single placenta. Two sets of twins, including 2 fetuses without SHR, had unfavorable courses. One fetus showed SHR and the other did not in 2 sets of twins, but both fetuses showed SHRs in a twin pregnancy. There was one intrauterine fetal death whose SHR was of a lower frequency and higher amplitude than the others.
