Uterine Malignancies Developing after a Long Term Use of lUCD's

Four cases of uterine malignancies developed after use of IUCD's: 3 cases after a long term use and one case after a short time use. In Case 1, sarcoma developed 20 years after application at the exact site where the IUCD (nylon ring) was placed and well corresponded to the size and shape of the IUCD and had also invaded the myometrium. Local chronic irritation as a causative factor is strongly suggestive. In Cases 2 and 3, endometrial cancer developed 13 and 14 years after the application of a Lippes loop. It seems injudicious to interpret the occurrence solely as an accidental coincidence. Loops after prolonged use could have played some role in the development of the endometrial cancer. In the fourth case endometrial cancer developed 3 months after the placing of a nylon ring in the uterus at the time of tuboplasty to permit easy extraction of the polyethylene tube which was inserted through the fallopian tube. In this case, in view of her age, it is possible that preexisting endometrial hyperplasia with undetected precancerous lesions might have been present when the polyethylene tube and nylon ring were inserted and subsequently evolved into malignancy. The authors emphasize the importance of awareness of malignancy in prolonged vaginal bleeding in IUCD users, and consider a short term and interrupted use of IUCD's safer and advisable, although any cause and effect relationship between IUCD and uterine malignancy is still a matter of pure conjecture.
