Summary Sixty‐six cases of transverse lie in singletons and twin pregnancies are reviewed to highlight the trend in management modality. Multiparity, premature rupture of fetal membranes, antepartum haemorrhage, intrauterine death and congenital uterine abnormality were the main aetiological factors. There were 4(6.1%) cases of ruptured uterus. Most of the patients had caesarean section, mainly lower segment irrespective of the viability of the fetus. Although 57.7% of the patients had puerperal complications in form of sepsis, wound breakdown, anaemia and postpartum haemorrhage, there was no maternal death. The corrected perinatal mortality was 203 per 1000. Booked patients had better fetal outcome with perinatal mortality of 83 per 1000 compared with 400 per 1000, in unbooked patients. We have advocated increased use of antenatal facilities by patients, early diagnosis and investigation of transverse lie. We believe maternal and fetal outcome will improve with liberal use of caesarean section.
