Primary Radical Hysterectomy and Bilateral Pelvic Lymphadenectomy for Carcinoma of the Uterine Cervix

Primary radical hysterectomies with bilateral lymph node dissections were carried out on 100 consecutive cases, of Stage 1 and II carcinoma of the uterine cervix from October 1961 to December 1969, representing one‐third of all Stage I and II admitted to the Philippine General Hospital for treatment. The operative mortality was 3%. The 5 year cure rates of 70.2% (26/37) for Stage I and 53.5% (15/28) for Stage II arc calculated from the 65% of patients who have completed a minimum follow‐up of 5 years. With negative lymph nodes, the cure rates are 78.1% and 64.7% for Stage I and II respectively. The complications encountered were pelvic and urinary tract infections 51%; uretero vaginal fistulae 5% and vesicovaginul fistulae 2%. All the deaths and most of the complications occurred in the first five years, of study. But with improvements in operative technique, pre‐ and post‐operative care of patients in the last 4 years less complication occurred and improvement of the results are expected.
