A case of chronic acral ulceration is described in a 54‐year‐old man with cryofibrinogenaemia. Initial management with prednisolone and azathioprine was unsuccessful in controlling the disease. Introduction of a programme of plasmapheresis in addition to treatment with cyclophosphamide and prednisolone has brought about marked improvement. (Aust NZ J Med 1991; 21: 444–446.)
