We report a case of congenital lobar emphysema requiring surgery in early adult life to relieve respiratory symptoms of fate onset. Regional lung function studies helped to predict the outcome of surgery which was successful in relieving symptoms. (Aust NZ J Med 1986; 16: 501–505.)
