Two cases of neuroleptic malignant syndrome (NMS) are presented. This rare pattern of neuroleptic drug toxicity is characterised by fever, muscular rigidity, akinesia and elevated plasma creatine kinase. The condition may be fatal and early recognition is essential. Evidence is presented that NMS is unrelated to malignant hyperpyrexia.
