This report documents a case of myelofibrosis with periostitis, initially involving the lower extremities and later the upper limbs and ribs. This was associated with fever, night sweats, marked bone pain and tenderness, prominent venous distension and a rapidly fatal course. There was no associated clubbing. Radiotherapy to the lower limbs resulted in both subjective and objective improvement, albeit shortlived.
