Summary: A patient with hypothermia due to acute barbiturate intoxication is presented. Features not usually present in the electrocardiograms of hypothermic patients suggested myocardial infarction. Hypothermia in temperate climates is uncommon but this diagnosis should be suspected when the characteristic electrocardiogram changes are seen.
