Summary: A boy aged seven years presented with a rapidly growing nodule of the thyroid. Histologically the nodule showed evidence suggestive of malignancy, the paranodular tissue showed marked hyperplasia. The remaining thyroid tissue was removed surgically and analysis of this revealed an isolated thyroglobulin deficiency. It is postulated that this deficiency led to the development of thyroid cancer.
