Summary: A young patient with systemic lupus erythematosus is described who developed a very acute exacerbation of the disease with fever, rash, arthralgia and severe toxaemia. Rapid deterioration occurred initially despite steroid therapy, and this was attributed to pulmonary microembolism. Features indicative of acute intravascular coagulation were demonstrated and, with continuing high dosage steroid therapy, the patient made a steady and complete recovery.
