USE OF LOCAL PERFORATOR FLAPS IN PARTIAL MASTECTOMY DEFECTS

Purpose: To demonstrate various possibilities of local pedicle perforator flaps use (mainly L‐ICAP and also A‐ICAP, LoTP and TAP flaps) for different types of partial mastectomy defects .Specific modifications are proposed to enhance aesthetic outcomes of these procedures. To determine suitability of these flaps as a method of choice for selected patients. Methodology: On‐going study – to‐date 17 patients. Patients with breast cancers suitable for breast conserving surgery but with post‐resection defects too large to be reconstructed with parenchymal flaps. Immediate reconstructions in all patients. Choice of flap dictated by site /size of the defect and the availability of local perforators (confirmed preoperatively by Doppler probe). Modifications are devised to maintain /re‐create aesthetic units of the breast. Detailed documentation including patients’ oncologic data and pre‐, intra‐ and post‐ operative photographs as well as follow‐up photographs after completion of radiation therapy. Results: These include: patients satisfaction, aesthetic results (using a panel of independent observers), complications, lessons learned and suggestions.
