The patient has ongoing pain following trauma or surgery, you wonder “Can it be a neuroma?”. What can be done? Despite re‐operation and burying the neuroma, the pain continues. Where now? Perhaps the peripheral stimulus is no longer important. The patient has developed “sensitisation” both peripherally and centrally such that further surgery at the site of the lesion may actually exacerbate the problem. A brief discussion of the concepts of peripheral and central sensitisation and possible management strategies will be delineated.
