Purpose:   Despite the growing number of patients with acquired brain injury with upper limb spasticity (ULS) in our community, hand surgery involvement in their management is rare and the literature on this topic is sparse. This presentation endeavours to promote more knowledge on the clinical assessment and surgical options of patients with ULS, including the use of Botulinum Toxin A as a pre‐operative diagnostic tool. Methodology:   The data of 20 patients operated at Royal North Shore Hospital and North Shore Private Hospital over the last two years are reviewed. Results:   The results of surgery and outcome measures are presented. Conclusion:   There is an important role for surgery in the functional and non‐functional spastic upper limb.
