The basic message is DON'T. Groin Hernia:   There is evidence that groin herniae in males are very safe to watch. Elective hernia repair therefore should be carefully considered. The risk of infection, of recurrence and of non surgical complications is high. Watchful waiting may well be the best option. In case of emergency a policy of “primum non nocere” may be the best option. Incisional Hernia:   Weight reduction prior to surgery results in fewer complications and happier surgeons! If operation is necessary in an emergency situation “Live to fight another day” may be the best motto. If operation cannot be delayed, the choice between open and laparoscopic surgery is difficult. Laparoscopic perhaps gives lesser pain but is more difficult. Open surgery is not easy and is associated with all the complications that operating in morbidly patients is associated with. Each case must be assessed and decisions made. Whatever decision is made, there is a large risk of complications, and the surgical procedure therefore must reflect the urgency of the situation.
