Aortic dissection presenting with acute lower limb ischaemia is not uncommon. However, stanford B dissection into a pre‐existing abdominal aortic aneurysm resulting in bilateral lower limb ischaemia has not been reported in the literature. 1 This patient underwent unsuccessful revascularisation of the lower limbs with an axillo‐bifemoral bypass procedure.
