The bronchoscopic appearance of a patient who developed acute respiratory distress following the insertion of a pulsion endo‐oesophageal tube is described. Intubation of the lower thoracic oesophagus causes cardiooesophageal incompetence, which is considered important in causing aspiration pneumonia. The selection of a suitably sized tube is important as a possible means of preventing tracheobronchial compression.
