A case of perianal Paget's disease is presented. The salient features of this disease include perianal irritation, a perianal rash, and usually, as in this patient, an associated underlying carcinoma. The disease is rare and has to be differentiated from malignant melanoma, Bowen's disease and transitional cell carcinoma. This patient was also remarkable since the associated malignancy was an equally rare carcinoma of anal gland origin.
