Acute obstructive uropathy, following an abdominal hysterectomy in a 45‐year‐old woman, had been temporarily circumvented by bilateral nephrostomy. Reconstruction of the upper urinary tracts, in the face of a small, defunctionalized bladder, was successfully accomplished by a Boari flap ureteroplasty and transureteroureterostomy.
