Flared nostril deformlty is often seen following repair of congenital cleft lip and palate. The basis and cause of the deformity are described. It is contended that skin shortage over the alar and lateral cartilages of the nose is an integral part of the deformity. The use of Wolfe grafts in association with alar repositioning is described, and an illustrative case is presented.
