A hepatic hydatid cyst presenting as an extraperltoneal cyst is reported, and the relevant literature is reviewed. The cyst, which had originated in the left lobe of the liver, had burrowed in between the anterior parietal peritoneum and the posterior rectus sheath, and had reached such an enormous size as to become adherent to the whole of the abdominal wall. Such a presentation has not been reported elsewhere for many years.
