Bilateral dilatation of the upper urinary tract in the adult male is not necessarily due to bladder neck obstruction requiring surgical treatment. It can be result of inflammatory disease, in which even it may well respond to conservative therapy. The case of a patient in this category is reported. Summary The presence of an insignificant amount of residual urine in combination with symptoms of prostatism and a radiologically demonstrated dilated upper tract should alert one to the possibility of these phenomena being of infective rather than of obstructive origin. A plea is made for the avoidance of hasty prostatic surgery and for the treatment of such patients on medical lines. A case report is presented to illustrate that this approach can be successful.
