A case of perforated acute appendicitis is described, presenting as intestinal obstruction in a premature infant 16 days after birth. The appendix was removed at laparotomy, and the infant made an uneventful recovery. A brief comment is made on the rarity of acute appendicitis in the premature newborn. The infant whose case is reported had been fed on a cow's milk mixture. It is suggested that this may have been a factor in the came of the appendicitis .
