Au unusual and perhaps unique case of carcinoma of the head of the pancreas is reported, which presented with true obstruction of the second part of the duodenum and did not in any way involve the major biliary or pancreatic ducts. It was treated by partial gastrectomy, duodenectomy and total pancreatectomy and splenectomy. Various aspects of the prognosis are discussed and correlated with series of pancreatic Carcinomata from the literature.
