Summary Five cases of aneurysmal bone cyst are presented. The aetiology, clinical signs, radiology and pathology are outlined. They illustrate the typical features of this lesion: a short history of a painful bony swelling which radiologically is eccentric, ballooned out and honeycombed. They also emphasize the protean clinical and radiological manifestations which present diagnostic difficulties. The lesion has masqueraded as a benign bone cyst, giant cell tumour, osteogenic sarcoma and even hydatid. The pathologist must have an adequate biopsy, for without this he may find it difficult to distinguish the cyst from other giant cell bone lesion. Complete resection or curettage is a satisfactory form of treatment.
