The case of a patient who survived necrotizing colitis is described in detail. The view is expressed that this condition is a distinct entity of obscure ætiology. Radical surgery, in the form of subtotal colectomy with double colostomy or ileostomy and sigmoidostomy, is advocated as offering the only hope of survival. Clinical features which should lead to suspicion of the diagnosis before operation are outlined, and the need for vigorous preoperative and intraoperative resuscitation is stressed.
