Summary The present day approach to limb injuries involving major vessels has been outlined. The importance of early exploration of all limbs rendered ischaemic by trauma in the region of major vessels has been stressed. Major vessel continuity, both arterial and venous, must be re‐established wherever possible. Case histories have been quoted to illustrate the principles in the management of acute and chronic vascular trauma.
