Summary 1. An example of accidental surgical ligation of the ureter with hydronephrotic atrophy is a low lumbar ectopic kidney is recorded. 2. The development of hydronephrosis following surgical ligation of the ureter is insidious. 3. Pain may not be a clinical feature until hydronephrotic atrophy is complete, and in the case recorded, three and a half years passed without symptoms or signs. 4. The presence of infection would appear to determine the onset of acute symptoms following surgical ligation of the ureter.
